    Prevention of orthodontic enamel demineralization: a systematic review with meta‐analyses

    Aim of this systematic review was to assess the efficacy of preventive interventions against the development of white spot lesions (WSLs) during fixed appliance orthodontic treatment. Nine databases were searched without limitations in September 2018 for randomized trials. Study selection, data extraction and risk of bias assessment were done independently in duplicate. Random-effects meta-analyses of mean differences (MDs) or relative risks (RRs) with their 95% confidence intervals (CIs) were conducted, followed by sensitivity analyses, and the GRADE analysis of the evidence quality. A total of 24 papers (23 trials) were included, assessing preventive measures applied either around orthodontic brackets (21 trials; 1427 patients; mean age 14.4 years) or molar bands (2 trials; 46 patients; age/sex not reported). Active patient reminders were associated with reduced WSL incidence on patient level compared to no reminder (3 trials; 190 patients; RR: 0.4; 95% CI: 0.31-0.64; Number Needed to Treat [NNT]: 3 patients), flat surface sealants were associated with reduced WSL incidence on tooth level than no sealant (5 trials; 2784 teeth; RR: 0.8; 95% CI: 0.63-0.95; NNT: 33 teeth), and fluoride varnish was associated with reduced WSL severity on tooth level (2 trials; 1160 teeth; MD: -0.32 points; 95% CI: -0.44 to -0.21 points). However, the quality of evidence was low according to GRADE, due to risk of bias. Some evidence indicates that active patient reminders and flat surface sealants or fluoride varnish around orthodontic brackets might be associated with reduced WSL burden, but further research is needed.     Adaptive Diversity of Beech Seedlings Under Climate Change Scenarios

    The ability of beech ( Fagus sylvatica L.) populations to adapt to the ongoing climate change is especially important in the southern part of Europe, where environmental change is expected to be more intense. In this study, we tested the existing adaptive potential of eight beech populations from two provenances in N.E. Greece (Evros and Drama) that show differences in their environmental conditions and biogeographical background. Seedling survival, growth and leaf phenological traits were selected as adaptive traits and were measured under simulated controlled climate change conditions in a growth chamber. Seedling survival was also tested under current conditions in the field. In the growth chamber, simulated conditions of temperature and precipitation for the year 2050 were applied for 3 years, under two different irrigation schemes, where the same amount of water was distributed either frequently (once every week) or non-frequently (once in 20 days). The results showed that beech seedlings were generally able to survive under climate change conditions and showed adaptive differences among provenances and populations. Furthermore, changes in the duration of the growing season of seedlings were recorded in the growth chamber, allowing them to avoid environmental stress and high selection pressure. Differences were observed between populations and provenances in terms of temporal distribution patterns of precipitation and temperature, rather than the average annual or monthly values of these measures. Additionally, different adaptive strategies appeared among beech seedlings when the same amount of water was distributed differently within each month. This indicates that the physiological response mechanisms of beech individuals are very complex and depend on several interacting parameters. For this reason, the choice of beech provenances for translocation and use in afforestation or reforestation projects should consider the small scale ecotypic diversity of the species and view multiple environmental and climatic parameters in connection to each other

    Business cycles in Greek maritime transport: an econometric exploration (1998–2015)

    Maritime transport has been a crucial input for the growth of the Greek economy given that the Greek fleet is one of largest merchant fleets in the world. However, the impact of the local and international business cycle on Greek maritime transport is inadequately researched, so far, in the literature. In this context, the present paper investigates the key determinants of maritime transport fluctuations in the three major ports of the Greek hinterland, taking into account a number of variables for the 1998–2015 time-span, capturing, at least partly, the global financial crisis and the local crisis, as well. To this end, various relevant quantitative techniques have been used, such as Granger causality, Dufour and Renault multistep causality and SURE system estimation. Our main finding is that Greek maritime transport traffic, as expressed through the cargo volumes of the three major ports of Piraeus, Volos and Thessaloniki, has not been influenced by the Greek business cycle, implying that the country’s maritime sector is practically independent of the macroeconomic conditions of the total economy. Clearly, future and more extended research would be relevant in the direction of applying the aforementioned approach to other EU countries of the Mediterranean

    Effect of simulated gastro-duodenal digestion on the allergenic reactivity of beta-lactoglobulin

    <p>Abstract</p> <p>Background</p> <p>Cow's milk (CM) allergy affects about 2% of infants. The allergenicity of dietary proteins, including those from CM, has been related to their digestibility although the generality of the link and its causality remains to be demonstrated. In this study we use an in vitro digestion system, to investigate the digestibility of β-lactoglobulin (blg) during gastrointestinal transit and to assess the impact of this process on blg allergenic reactivity in CM allergic children.</p> <p>Methods</p> <p>Blg digesta were prepared using an <it>in vitro </it>digestion protocol simulating either gastric digestion alone or followed by duodenal digestion with or without phosphatidylcholine (PC). Biochemical analysis of blg digesta was performed by SDS-PAGE and their concentration was measured by a sandwich ELISA. Assessment of their allergenic reactivity was done <it>in vitro </it>by EAST inhibition, specific basophil activation (basotest) and lymphocyte proliferation (PCNA-flow cytometry) assays using sera and cells from patients allergic to blg and <it>in vivo </it>by skin prick testing (SPT) of these patients.</p> <p>Results</p> <p>Blg was only broken down to smaller peptides after gastro-duodenal digestion although a sizeable amount of intact protein still remained. Digestion did not modify the IgE binding capacity of blg except for gastro-duodenal digestion performed in the absence of PC. These results are consistent with the quantity of intact blg remaining in the digesta. Overall both gastric and gastroduodenal digestion enhanced activation of sensitized basophils and proliferation of sensitized lymphocytes by blg. However, there was a tendency towards reduction in mean diameter of SPT following digestion, the PC alone during phase 1 digestion causing a significant increase in mean diameter.</p> <p>Conclusions</p> <p>Digestion did not reduce the allergenic reactivity of blg to a clinically insignificant extent, PC inhibiting digestion and thereby protecting blg allergenic reactivity. SPT reactivity was reduced compared to blg immunoreactivity in <it>in vitro </it>tests.</p

    The Cues and Care Trial: A randomized controlled trial of an intervention to reduce maternal anxiety and improve developmental outcomes in very low birthweight infants

    Abstract Background Very low birthweight infants are at risk for deficits in cognitive and language development, as well as attention and behaviour problems. Maternal sensitive behaviour (i.e. awareness of infant cues and appropriate responsiveness to those cues) in interaction with her very low birthweight infant is associated with better outcomes in these domains; however, maternal anxiety interferes with the mother's ability to interact sensitively with her very low birthweight infant. There is a need for brief, cost-effective and timely interventions that address both maternal psychological distress and interactive behaviour. The Cues and Care trial is a randomized controlled trial of an intervention designed to reduce maternal anxiety and promote sensitive interaction in mothers of very low birthweight infants. Methods and design Mothers of singleton infants born at weights below 1500 g are recruited in the neonatal intensive care units of 2 tertiary care hospitals, and are randomly assigned to the experimental (Cues) intervention or to an attention control (Care) condition. The Cues intervention teaches mothers to attend to their own physiological, cognitive, and emotional cues that signal anxiety and worry, and to use cognitive-behavioural strategies to reduce distress. Mothers are also taught to understand infant cues and to respond sensitively to those cues. Mothers in the Care group receive general information about infant care. Both groups have 6 contacts with a trained intervener; 5 of the 6 sessions take place during the infant's hospitalization, and the sixth contact occurs after discharge, in the participant mother's home. The primary outcome is maternal symptoms of anxiety, assessed via self-report questionnaire immediately post-intervention. Secondary outcomes include maternal sensitive behaviour, maternal symptoms of posttraumatic stress, and infant development at 6 months corrected age. Discussion The Cues and Care trial will provide important information on the efficacy of a brief, skills-based intervention to reduce anxiety and increase sensitivity in mothers of very low birthweight infants. A brief intervention of this nature may be more readily implemented as part of standard neonatal intensive care than broad-based, multi-component interventions. By intervening early, we aim to optimize developmental outcomes in these high risk infants.     Management of asthma in childhood: study protocol of a systematic evidence update by the Paediatric Asthma in Real Life (PeARL) Think Tank

    IntroductionClinical recommendations for childhood asthma are often based on data extrapolated from studies conducted in adults, despite significant differences in mechanisms and response to treatments. The Paediatric Asthma in Real Life (PeARL) Think Tank aspires to develop recommendations based on the best available evidence from studies in children. An overview of systematic reviews (SRs) on paediatric asthma maintenance management and an SR of treatments for acute asthma attacks in children, requiring an emergency presentation with/without hospital admission will be conducted.Methods and analysisStandard methodology recommended by Cochrane will be followed. Maintenance pharmacotherapy of childhood asthma will be evaluated in an overview of SRs published after 2005 and including clinical trials or real-life studies. For evaluating pharmacotherapy of acute asthma attacks leading to an emergency presentation with/without hospital admission, we opted to conduct de novo synthesis in the absence of adequate up-to-date published SRs. For the SR of acute asthma pharmacotherapy, we will consider eligible SRs, clinical trials or real-life studies without time restrictions. Our evidence updates will be based on broad searches of Pubmed/Medline and the Cochrane Library. We will use A MeaSurement Tool to Assess systematic Reviews, V.2, Cochrane risk of bias 2 and REal Life EVidence AssessmeNt Tool to evaluate the methodological quality of SRs, controlled clinical trials and real-life studies, respectively. Next, we will further assess interventions for acute severe asthma attacks with positive clinical results in meta-analyses. We will include both controlled clinical trials and observational studies and will assess their quality using the previously mentioned tools. We will employ random effect models for conducting meta-analyses, and Grading of Recommendations Assessment, Development and Evaluation methodology to assess certainty in the body of evidence.Ethics and disseminationEthics approval is not required for SRs. Our findings will be published in peer reviewed journals and will inform clinical recommendations being developed by the PeARL Think Tank.PROSPERO registration numbers CRD42020132990, CRD42020171624.</p
